Mr. Johnson died in December 2009 of liver cirrhosis due to alcoholism, a disorder he concealed from everyone.
His parents wish that all who have a substance use disorder can access treatment. They hope this page will help others avoid what happened to their son.

2—Type your city, state, and zip code.
3—Select radius (those treatment facilities that are 1, 5, 10, 20, 50, or 100 miles from your address). Click Continue.
4—Most treatment facilities within the distance you selected will appear on a map, with the closest first, the next closest second, and so on.
5—Scroll down the page to obtain information about each facility’s name, address, phone, distance, and a map showing its location.
6—Also included is information about who each facility treats (men, women, adolescents, persons with HIV/AIDS, and so on), kinds of treatment services offered, forms of payment accepted, and patient assistance offered.
7—Please take the time to find the right treatment facility for you. Don’t be afraid to call those that look promising and ask questions. Remember, you are the patient and you know better than anyone else what your treatment needs are.
8—Good luck! We know you can do it and wish you all the best. Remember, relapse is part of treatment. If you relapse, don’t despair. Go back for more treatment and keep at it. Persistence will get you there!
